TargetCare utilizes the most comprehensive and advanced Clinical Health Assessment tool in use today. Developed by an award-winning life science organization, the patented and proprietary predictive modeling program enables TargetCare to quantify an individual’s risk of developing chronic disease. Knowledge is the foundation of meaningful change. It is through TargetCare’s unique Clinical Health Assessment (CHA) and Personal Health Consult that participants are empowered to learn about their individual health risks and lifestyle choices that most influence their risk. Unlike many of the fully self-reported health risk questionnaires that exist, our CHA requires lab results, clinical measurements and one-on-one consultation with a health professional.
Organization and Promotion TargetCare account management team will offer assistance in planning a worksite CHA event. We will create sample roll-out letters, prepare promotional materials, and supply all the necessary tools to schedule participants. We will also help design incentives that motivate employees to participate while ensuring privacy and confidentiality throughout the process.
TargetCare staff will direct and conduct the worksite CHA event which will include:
Assisting participants in completing the questionnaire
Taking biometric measurements such as blood pressure, height and weight
Collecting blood samples of each participant
Comprehensive Blood Screening The base-line blood tests that independent laboratories perform on each sample are typical of those performed by a physician at an annual physical, and include many tests that extend beyond a typical worksite screening. Our comprehensive test uses 26 indicators ranging from liver and kidney function to hydration, electrolyte balance, blood sugar, and blood cholesterol levels. Optional blood tests may be added to the initial screening at the employer’s request at additional costs.
Personal Health Consult
From the results of the Clinical Health Assessment, TargetCare prepares an individual Chronic Disease Risk Summary for each participant. This report shows a breakdown of the participant’s risk factors and compares them to “desirable” measures. The report also illustrates the actual risk of development of each targeted chronic disease and how each participant compares with his/her peer group - those with same age and gender. The report is an educational tool to point out areas of concern and to show how to reduce overall risk of developing chronic disease.
The personal scorecard is presented to each participant at a pre-scheduled one-on-one follow-up meeting at the work-site with a member of our healthcare staff. The one-on-one consult is a vital aspect of TargetCare’s philosophy. The consult ensures that each participant fully understands his/her results and encourages questions. While we urge every participant to have a relationship with a primary care physician, we realize that the Personal Health Consult may represent the only exposure that some participants have to a health provider. Our illustrative reports and face-to-face feedback help each participant understand their unique health risk, thereby encouraging personal accountability for their lifestyle and health behaviors. It is also during this time that our provider discusses “next steps” which may include:
Guiding participants to other medical resources or EAP
Explaining employer incentive programs
Educating participants on Reasonable Alternative Standards for outcome-based incentive programs
Corporate Health Profile
TargetCare meets with our client companies annually to present the Corporate Health Profile. This report summarizes the aggregate data collected from the participants in a confidential and de-identifiable format. The report details trends in the population as compared to national averages and indicates predicted cases and associated costs of chronic diseases. It also compares current year data to previous year’s results. The Corporate Health Profile reveals valuable information about a company’s projected health risk and empowers management to employ strategies to mitigate this risk.
