Assessing the Health of a Population
TargetCare Clinical Health Assessment requires lab results, clinical measurements and one-on-one consultation with a health professional. We utilizes the most comprehensive and advanced Clinical Health Assessment tool in the industry today. With this tool, TargetCare is able to stratify the risk burden of the employer's population and quantify an individuals risk of developing chronic disease.
Health Risk QuestionnaireThe first step in our screening process begins with the participants completing a health risk questionnaire prior to the blood draw and biometric measurements. The questionnaire addresses an individual’s medical history, lifestyle and behavior, as well as emotional health.

Blood Draw & Biometric MeasurementsOur screening process includes a base-line blood test that independent laboratories perform on each sample and are typical of those performed at an annual physical. The test uses 26 indicators ranging from liver and kidney function to hydration. Optional tests are available to be added at the employer's request.

Personal Health ConsultFollowing the completion of the assessment and blood screening, TargetCare prepares an individual personal scorecard for each participant that summarizes the risk of developing chronic diseases. This educational tool is reviewed with a provider in a one-on-one personal health consult. The consult ensures that each participant fully understands their unique health risk, and encourages personal accountability for lifestyle and health behaviors.
